Research one of the aircraft incidents listed below. These are real-world events that we will be referring to in class over the next few days, so this activity will help build your background knowledge.

Instructions

Using reliable sources (e.g. news articles, official investigation reports, airline or regulator statements), answer the following in your notes:

What happened?
- Provide a brief overview of the incident
- Identify which phase of flight it occured in (e.g. taxi, take-off, climb, cruise, descent, landing)
- Explain who was involved (flight deck, cabin crew, passengers, ground staff, ATC, etc.)
What were the key learnings?
- Why did it go well or not so well, and what contributed to that outcome?
- What could have been done differently, and why?
- What did the crew, airline or industry learn from this event?
